Expressions of interest for the provision of a day service 'Remember Me' for people living with Dementia
Swansea Social Services are inviting organisations who have experience of working with carers and providing support to people living with Dementia to provide a Day Service for people living with Dementia. The anticipated start date of this service is the week of 2 April 2012.
Service to be provided:
The provision of 1 member of staff and at least two volunteers to cover a five hour session in Norton Lodge Day Centre every Thursday (including time associated with transport) – with the potential to expand to other sites across City and County of Swansea in the future.
A commitment to work with up to 10 service users in each session, along with their carers.
A commitment to provide gender appropriate staff and volunteer support.
Participation in home visits with in-house staff to support the development of Strengths Based Support Plans.
A commitment to ensuing continuity of worker both in terms of staff and volunteer provision.
The provision of transport to and from the day centre for Service Users who cannot be transported either by their family / friends or community transport schemes.
Broad Criteria:
We are looking for organisations with the following expertise and qualities:
- Experience of supporting carers
- Experience of providing support to people living with Alzheimer's / Dementia
- Experience of recruiting and supporting volunteers
- Experience of providing day care / support
- The capacity to provide a sustainable service
- The ability to grow and develop the service
Process:
To register an interest and receive an Expressions of Interest Information Pack your organisation must:
1. Have experience of supporting carers.
2. Demonstrate experience of providing support to people living with Dementia.
3. Experience of recruiting and supporting volunteers.
NB: PLEASE DO NOT COMPLETE THE QUESTIONNAIRE IF YOU DO NOT HAVE THIS EXPERIENCE AS YOUR ORGANISATION WILL NOT BE CONSIDERED.

The closing date to submit your completed questionnaire is 8.30am on Monday, 9th January 2012. Please note that Questionnaires received after this date will not be considered.
