Age Cymru Swansea Bay - Visiting Assistant, Hospital Discharge Service

Salary: £13,184 p.a. pro rata - with a contributory pension scheme, 5%
Employer - 5% Employee after 3 months service

Location: Based at Sun Alliance House, 166-167 St Helen's Road, Swansea to cover City & County of Swansea

Hours: 16 hours per week. Normal working hours are Monday 10.00 am to 4.00 pm, Wednesday and Friday 10.00 am to 3.00 pm. Some out of hours work may be required, for which time off in lieu will be given

Annual Leave: 26 days - pro rata and customary bank holidays

Reporting to: Assistant Managers, Hospital Discharge Service

Travel Expenses: 45p per mile plus 5p per passenger

This is until 31 March 2012 in the first instance

The post holder will provide short-term low-level practical and emotional support to older persons within the City and County of Swansea, on their return home from hospital.
