EMERGENCY TREATMENT YOU CAN CARRY

The Crisis Information Card launched originally in 2005 by the Swansea Mental Health Forum has been redesigned to reflect the new ABMU Mental Health Forum.

This card is for use in a mental health emergency when its carrier may have difficulty expressing their wishes. In these circumstances the carrier's nominee will be contacted to be their advocate. They can call their nominee immediately, to provide appropriate support, such as childcare, domestic arrangements and notifying friends and relatives. It can also make police, medical staff and others assisting aware of the user's underlying condition.

The Card have proven very popular with service users and can be folded for easy use. The image below shows it unfolded:
