Cwmtawe Cluster invites proposals from organisations to independently evaluate the Cwmtawe Social Prescribing Project and produce recommendations, which will help to inform future working.

Cwmtawe Cluster is a network of GP Practices (Clydach Primary Care Centre, Llansamlet Surgery, Llys Meddyg Surgery, New Cross Surgery, Strawberry Place Surgery) that have come together in order to share services, expertise an pool resources for the local community. Community networks provide a way in which GP Practices and a range of health, social care and voluntary sector professionals will work more closely together. One of the key areas of focus for the cluster has been the establishment of a social prescribing service, which commenced in October 2017. Social Prescribing is a service connecting people to non-medical sources that will enable them to improve their health and well being.

The deadline for receipt of proposals is 31st December 2017.
