Cwmtawe Cluster logo June 2019The Cwmtawe Cluster is a group of three GP practices working together with community nursing, dentists, opticians, community pharmacists the voluntary sector and the local authority. Together the cluster provides care for around 44,000 people in the Clydach, Morriston and Llansamlet areas of Swansea.

NB – Whilst this post is within the public rather than third sector, we have included it in the SCVS Third Sector vacancies section as there will be large areas of overlap with third sector providers in this role.

Triage and Wellbeing Practitioner (CTWP)

Hours: Part time, 2.5 days per week

Salary: £13,500 actual

Fixed Term: Until March 31st 2022

Based: TBA (but will work at all 3 practices across the Cluster)

The CTWP will offer a psychologically informed initial point of contact and access to mental health and wellbeing support as well onward referral to Therapeutic intervention eg: Counselling.

The CTWP will support individuals of all ages presenting to the GP with mild to moderate mental health issues.

The post holder will support and signpost individuals of all ages that fall outside the remit of the LMPHSS, CAMHS or secondary mental health services.

The post holder will increase individual’s self-esteem and confidence and reduce the likelihood of individuals needing further help for their mental health.
