SCVS Logo - no wordsThe post holder will be responsible for the delivery of the Cwmtawe Dementia Support Project, which aims to reduce isolation and improve the wellbeing of people living with Dementia and their carers within the Cwmtawe Cluster, which will be achieved through the provision of a Dementia specific Social Prescribing Project and an awareness Cwmtawe Cluster logoraising and training package.

  • Accountable to: Health and Well Being Partnerships Manager
  • Reporting to: SCVS and the Cwmtawe Cluster
  • Hours: 28 hours (initially to March 2019)
  • Location: SCVS and Cwmtawe GP Cluster
  • Salary: NJC Scale point 28 - £25,463 per annum (Pro Rata)

Funded by the Western Bay Dementia Action Plan Integrated Care Funding 18/19, the project is a partnership between Swansea Council for Voluntary Service, Cwmtawe GP Cluster and ABMU Health Board.

The Cwmtawe GP Cluster is a group of three GP Practices (Cwmtawe Medical Group (Clydach Primary Care Centre, Sway Road Surgery and New Cross Surgery), Llansamlet Surgery and Strawberry Place Surgery) that have come together in order to share services, expertise and pool resources for the local community.
