*This project has now ended, however, some of the groups have become self sustaining - please contact Amy or Lauren at SCVS on the details below if you have any questions or would like further information*

Enabling patients and carers to become more involved in the design and delivery of health and social services by influencing decision making and ensuring that services provided are developed to meet local needs.

The project helps:

  • To raise awareness of services that are available locally and how they can be accessed.
  • Patients and carers to share their views on the services that they currently receive or that they would like to receive.
  • Members to work with service providers to develop and deliver an improved service and support.

"Services" mentioned include primary care such as GP practices, pharmacies, optometrists, dentists and other walk-in services, Social Services and the Voluntary Sector.

A Patient and Carer Participation group has been set up in each of the five GP Community Network areas of Swansea:

  • Bay
  • City
  • Cwmtawe
  • Llwchwr
  • Penderi

To become a member of a group, you must be registered at a GP practice within the area. Members meet up every eight weeks. To encourage patients and carers to become members expenses can be paid and transport can be arranged. The project can speak with individuals around child care support and replacement care if this is required for an individual to attend a meeting.

Whilst the project has now ended, some of the groups have now become self sustaining. For more information please contact either Amy or Lauren at SCVS on 01792 544000 / [email protected]

Patient Carer Participation Groups were delivered by SCVS until 2017.
