Cwmtawe Pathway Service wins 2023 NHS Wales award!

The service which helps people with complex health and well-being needs while taking pressure off GPs, has now received national recognition after winning at the 2023 NHS Wales Awards.

Project Co-ordinator, Cara Lougher:

We are absolutely thrilled to announce that the Cwmtawe Pathway Service has won the NHS Wales Award (2023) in the category of ‘Delivering Person Centred Care’.
The service has provided accessible, meaningful and long term support to those who need it most (adults with multiple unmet and complex needs).

We are overjoyed that our approach and model has been recognised and we hope that the critical insights learnt from the project will help to enhance future provision in the region.

I would like to say a huge thank you to our funders and colleagues in the Cwmtawe Cluster for their commitment to fund this initiative, along with thanks to our colleagues in the Health Board, Local Authority and Third Sector for their collaboration and support as well.

We are really delighted to have won the award, congratulations to Cara and all the team who support the initiative and have helped to develop the pathway. Evaluation has demonstrated the really positive outcomes for those people who have been supported.

The service offers support to those struggling with substance misuse, mental health and domestic abuse, including sexual violence. Its support can also extend to family members affected by these issues.

It provides patient-centred care by working with each person to agree meaningful interventions that tackle their unmet needs or issues.

The pilot service was introduced in 2021 after mental health, domestic abuse and substance misuse were identified as three areas where the cluster – which covers Morriston, Clydach and Llansamlet – wanted to provide additional services.
