New Ways to Wellbeing

Swansea's Social Prescribing Service. SCVS is working in partnership with the 5 SBUHB GP clusters across Swansea to deliver a Social Prescribing Service – ‘New Ways to Wellbeing’.

We have come together in order to share services and expertise. This partnership aims to promote wellbeing for individuals and encourage positive engagement in the local community.

What is Social Prescribing?

Social prescribing is a service connecting people to non-medical sources that will enable them to improve their health and wellbeing. These sources can include activities, opportunities, or connections to community groups and voluntary organisations. *

This service can help those who may be feeling isolated or disconnected for reasons such as:

  • Bereavement
  • Chronic illness
  • Carer responsibilities
  • Long term absence from work
  • Mild mental health problems 

It is not an emergency or acute service, staff are not medically trained and patients take an active role in co-producing plans and options to improve their wellbeing.

The Social Prescribing Link Worker provides a connection between the GP, the patient, community projects, and third sector organisations. Social prescribing complements the GP service in taking a holistic view of a patient’s health and wellbeing by supporting people to re-engage with their community.

This service is open to anyone over 18 who is registered with any of the GP surgeries within the 5 SBUHB Clusters in Swansea:

Bay Cluster - Gower Medical practice, Kings Road Surgery in Mumbles, Sketty and Killay Medical Centre, St Thomas and West Cross Surgeries, The Grove Medical Centre, University Health Centre, Uplands and Mumbles Surgery

City Cluster - Brunswick Health Centre, Greenhill Medical Centre, Abertawe Medical Partnership, Kingsway Surgery, Mountain View Health Centre, Nicholl Street Medical Centre, SA1 Medical Centre, The Harbourside Health Centre

Cwmtawe Cluster – Cwmtawe Medical Group (including Clydach, New Cross and Sway Road surgeries), Strawberry Place Surgery, Llansamlet Surgery

Llwchwr Cluster - Estuary Group Practice (including Gowerton Medical Centre and Penybryn Surgery), Princess Street Surgery, Talybont Surgery, Ty’r Felin Surgery

Penderi Cluster - Brynhyfryd Surgery, Cheriton Medical Centre, Cwmfelin Medical Centre, Fforestfach Medical Group (including Dr Bensusan surgery and Dr Powell Surgery) and Manselton Surgery

Referrals can be made by your GP or any health professional in your surgery. The Social Prescribing Link Worker will then contact you to arrange an appointment where they will listen and explore what your needs might be and provide information on what is available in the local and wider community to help you find your way back to wellbeing.

The Link Worker can also discuss and address any potential problems or barriers to you accessing these options and help you where appropriate.

You can expect to see the Social Prescribing Link Worker a minimum of 3 times, and have the opportunity to find your own way to wellbeing. Appointments are available at various times and days.
