Clusters are made up of groups of GP practices, dentists, community pharmacies, local opticians, community nurses, allied health professionals, mental health professionals and the voluntary sector. They work together to pool resources and share best practice in a bid to help you remain fit and healthy, and to improve the way you are cared for if you do become unwell. Wherever possible, the cluster will try to accomplish this in the heart of your community, sparing you the trouble of having to travel to hospitals or central clinics. Below is a summary document per cluster which sets out the local population context, health priorities for 2026/27, available resources and opportunities for partnership working with the third sector, to improve health and wellbeing across the community: Afan Cluster SummaryBay Cluster SummaryCity Cluster SummaryCwmtawe Cluster SummaryLlwchwr Cluster SummaryNeath Cluster SummaryPenderi Cluster SummaryUpper Valleys Cluster Summary Third Sector Collaborative Lead Biographies Meet our newly appointed Third Sector Collaborative Leads (currently recruited to 6 of the 8 clusters).
